Helios Veterinary Vision (“Helios”, “we”, “us”) is a veterinary ophthalmology referral practice at 1760 Dundas St E, Mississauga, ON L4X 1L8. We respect your privacy and handle personal information in line with Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA). This policy explains what we collect through this website and in the course of caring for your pet, why we collect it, and the choices you have.

Information we collect

When a veterinarian refers a patient

Referrals come to us from veterinarians, usually through the referral form on this website. A referral may include:

the referring veterinarian’s name, clinic, address, phone, fax, email address and, optionally, CVO licence number;
the client’s (pet owner’s) name, phone numbers and email address;
the patient’s details, such as name, species, breed, sex, age and weight;
clinical information, such as exam findings, test results, medications, medical history and uploaded medical records, bloodwork and images.

The referring veterinarian confirms that they are able to share the client’s contact details and the patient’s records with us for the referral. To keep the form secure, we confirm the referring clinic’s email address with a one-time code before a referral can be sent.

When you contact us or visit

If you call, email or visit us, we collect the information you choose to give us (for example your name, contact details and your pet’s information) so we can respond, book appointments and provide care. As with any veterinary practice, we keep medical records about the patients we see.

When you apply for a job

If you apply for a position, we collect the information in your application (such as your résumé and contact details) to assess it and contact you about it.

When you use this website

Like most websites, our web server records basic technical information, such as your IP address, browser type and the pages you visit, to keep the site running and secure. The referral form also uses Cloudflare Turnstile to check that it’s being used by a person rather than an automated program. We don’t use advertising or analytics tracking. How we use information

to review referrals, contact clients to book appointments, and provide veterinary care;
to keep the referring veterinarian informed about their patient’s care, as they request on the referral;
to respond to questions and requests;
to process job applications;
to keep our website and records secure, and to meet our legal and professional obligations, including those of the College of Veterinarians of Ontario.

We don’t sell or rent personal information, and we don’t use it for advertising.

How we protect referral information

Information sent through the referral form is encrypted in transit and stored encrypted on our website.
Email notifications about new referrals never include client or patient details or records. Our team signs in to view them.
Only authorized Helios team members can view referrals.
Once a referral has been moved into our clinic’s practice management system, it’s removed from the website. Any copy left on the website is permanently deleted no later than 90 days after it was submitted.

When we share information

We share personal information only as needed to provide our services, for example with:

the referring veterinarian and other veterinary professionals involved in your pet’s care;
service providers who help us operate, such as our website host, email provider, practice management software, Cloudflare (form security) and Google (the map on our Contact page). They may only use the information to provide their services to us;
others where required or permitted by law, such as a regulator or court.

Some of these providers may store information outside Canada, where it may be subject to the laws of that country.

How long we keep information

Medical records are kept for as long as required by the College of Veterinarians of Ontario and applicable law. Referral submissions are removed from the website as described above. Other information is kept only as long as needed for the purpose it was collected, then securely deleted.

Your choices and rights

You may ask to see the personal information we hold about you, ask us to correct it, or withdraw your consent to certain uses (which may affect our ability to provide care). To make a request, contact us using the details below. If you have a concern we haven’t resolved, you may contact the Office of the Privacy Commissioner of Canada.

Changes to this policy

We may update this policy from time to time. The date at the top shows when it was last changed.
